Please read the information below carefully, as it forms an agreement between us. This document sets out how we will work together and your rights and responsibilities as a client.
This Terms of Service document is intended to make you fully aware of your rights and responsibilities as a client, and my responsibilities as your Practitioner Psychologist, including my professional obligations regarding confidentiality. At our first session I will set time aside to discuss this agreement with you and answer any questions. This contract is voluntary and you can end it at any time.
I am Dr Lisa Jane Stuart, a Principal Clinical Psychologist and accredited Cognitive Analytic Therapy (CAT) Practitioner. Practitioner Psychologist is a protected title regulated by the Health and Care Professions Council (HCPC).

I hold professional indemnity insurance. Details are available upon request.
I hold a Doctorate in Clinical Psychology and an MSc in Forensic Psychology, and have 18 years of experience in applied psychology. I have published PhD research in Pathological Demand Avoidance (PDA) and anxiety in children and adolescents. LJS Psychology is my independent private practice, operating entirely separately from any NHS employment.
I am an accredited CAT Practitioner. Cognitive Analytic Therapy is the primary framework within which I work. CAT is a structured, collaborative and time-limited form of psychotherapy that helps you identify and understand patterns in your thoughts, feelings, behaviours and relationships.
I also draw on CBT, DBT, Compassion Focused Therapy, trauma-informed and attachment-based approaches, and systemic and narrative approaches as appropriate to your individual needs.
Free consultation: I offer a free 15-minute consultation to explore whether we are a good fit to work together. No commitment is required.
Assessment: The initial assessment involves exploring your presenting difficulties and relevant history. We will develop a shared formulation and agree goals and an estimated number of sessions. Assessment may take more than one session.
Sessions: Sessions are 50 minutes, offered weekly or fortnightly, online or in person. The course of therapy is regularly reviewed. You may end therapy at any time, though at least one planned ending session is recommended.
All information you share is treated as strictly confidential. Confidentiality may be broken in limited circumstances: if you make serious threats to harm another person; if I have serious concerns about your safety; if you disclose information about serious crimes; or if required by a court order. Where possible I will discuss any breach with you first.
I receive regular clinical supervision and may discuss aspects of our work in anonymised form with my Clinical Supervisor, who is also bound by professional confidentiality standards.
ICO Registration: [XXXXXX]. All data is held in accordance with the Data Protection Act 2018 and UK-GDPR. Written notes are stored in a locked cabinet. Electronic records are on an encrypted, password-protected device. Records are retained for 7 years following the end of therapy.

Session fees are as follows:
Fees are reviewed annually with at least three calendar months' notice of any changes. Payment is due by bank transfer no later than 24 hours before each session.
Bank transfer details: LJS Psychology, Account: [XXXXXX], Sort code: [XX-XX-XX]. Please use your surname and appointment date as the reference. I will never notify you by email alone that bank details have changed.
If I need to cancel, you will not be charged and any payment already made will be refunded in full.
I do not offer an emergency or crisis service. If you need urgent support please contact your GP, your local A&E, Samaritans (116 123, free, 24/7), Mind (0300 123 3393), or your local NHS crisis team.

By signing the Consent to Treatment form (provided separately) you confirm that you have read and understood this Terms of Service document and consent to receiving psychological therapy from Dr Lisa Stuart at LJS Psychology. A copy of the consent form is available on request.
